Procedure: vaccune when Diphtheria-tetanus (dT)* 12 & 24 months Poliomyelitis* 12 & 24 months Haemophilus influenza type B 12 & 24 months 23-valent pneumococcal 12 & 24 months 4-valent meningiococcal 12 & 24 months Influenza seasonal Measles/mumps/rubella 24 months# varicella-zoster contraindicated * diptheria, pertussis, tetanus (DPT) in children < 7 years of age; inactivated poliovirus vaccine; live virus vaccine is contraindicated # in patients without graft vs host disease
