Indications: - Trypanosoma brucei gambiense & rhodesiense, Stage I (normal CSF) Dosage: 1) 100-200 mg IV test dose to detect hypersensitivity 2) adults: 1 g IV on days 1,3,7,14 & 21 3) children: 20 mg/kg/day (max 1 g) IV on days 1,3,7,14 & 21 4) slow IV infusion of freshly prepared 10% aqueous solution Contraindications: patients with renal insufficiency Pharmacokinetics: - does not penetrate blood-brain barrier Adverse effects: 1) transient proteinuria is common 2) renal failure 3) fever 4) photophobia 5) pruritus 6) arthralgias 7) skin eruptions 8) immediate, severe, potentially fatal hypersensitivity reaction (1 in 20,000) a) nausea/vomiting b) shock c) seizures Monitor: 1) urinalysis 2) discontinue if proteinuria or RBC in urine
