sestamibi (Cardiolite)
Tc-99-labeled sestamibi is used in myocardial perfusion & viability studies. Sestamibi is inefficiently taken up by the myocardium, but is efficiently retained without significant redistribution. Generally 25 mCi of Tc-99 sestamibi is used. 1/2 life of Tc-99 is 6 hours.
After injection, the highest Tc-99 sestamibi concentration is initially found in the gallbladder & liver, followed by heart, spleen & lungs. Within the 1st hour, the liver activity decreases, with excretion into the biliary system. The maximum concentration in the gallbladder is reached at 1 hour. At 1.5 hours, the count density is higher in the heart than immediately adjacent organs.
