Indications: - blood pressure screening in - adults >=18 years of age with office-based blood pressure measurement [5] - annual screening for hypertension in adults >= 40 years & for adults at increased risk for hypertension (Black persons, overweight or obese) [5] - screening every 3-5 years for most young adults 18-39 years [5] Management: - office blood pressure (BP) measurements should be repeated (see blood pressure) - ambulatory blood pressure should be used to confirm high blood pressure before a patient is diagnosed with hypertension [3,4] - 5-65% of patients with high blood pressure readings in the office are normotensive on ambulatory monitoring [3] - home blood pressure monitoring may be alternative [3,4] - a diagnosis of hypertension should be based on average of >=2 elevated systolic BP &/or diastolic BP obtain on >= 2 occasions (MKSAP19) [6] - the optimal screening interval is not known [1] - insufficient evidence to recommend screening for hypertension in asymptomatic children & adolescents [2,5] - obtain blood pressure measurements outside of the clinical setting for diagnostic confirmation before starting treatment [5]
