rimexolone (Vexol)

Tradenames: Vexol. Indications: 1) treatment of inflammation after ocular surgery 2) treatment of anterior uveitis Contraindications: 1) epithelial Herpes simplex keratitis (dendritic keratitis) 2) vaccinia, varicella & most other viral diseases of the cornea & conjunctiva 3) mycobacterial infection of the eye 4) fungal diseases of the eye 5) acute, purulent, untreated infections which might be masked or enhanced by glucocorticoids Caution: 1) prolonged use has been associated with development of corneal or scleral perforation & posterior subcapsular cataracts 2) effectiveness & safety not established in children Dosage: ophthalmic: 1 drop every 1-6 hours (1% suspension) Suspension: 1% (5 mL, 10 mL) Pharmacokinetics: 1) absorption through aqueous humor 2) any absorbed drug is metabolized in the liver 3) elimination in urine & feces Monitor: 1) intraocular pressure 2) periodic examination of the lens with prolonged use Adverse effects: 1) not common (1-10%) - temporary mild blurred vision 2) uncommon (< 1%) - stinging, burning of eyes, corneal thinning, increased intraocular pressure, glaucoma, damage to the optic nerve, defects in visual acuity, cataracts, secondary ocular infection
