right middle lobe syndrome
Recurrent atelectasis associated with isolated bronchiectasis of the right middle lobe of the lung.
Etiology:
1) compression of right middle lobe by lymph nodes
2) acute angle of right middle lobe origin
3) narrow opening of bronchus
4) long bronchus
5) lack of collateral ventilation
6) Mycobacterium avium complex (Lady Windermere syndrome)
Clinical manifestations:
1) recurrent bronchiectasis & atelectasis of right middle lobe
2) many patients are asymptomatic
Radiology:
1) diagnosis is frequently made as incidental finding on chest X-ray
2) band like opacity seen on lateral film extending from hilum to anterior chest wall at diaphragm
