- periodically monitor chronic health problems
- assess acute medical problems or changes in resident function
- implement appropriate measures
- communicate with the interdisciplinary team, the resident & the family regarding new diagnosis &/or treatment plans
- periodic medication reconciliation
- optimize quality of life & function
- special attention to cognition, mobility, falls, nutrition, skin integrity, & continence
- determine each resident's decision-making capacity & assist in establishing advance directives
- physically attend to each resident with documentation in the medical record in accordance with federal & state guidelines
