pyridoxine neuropathy; megavitamin-B6 syndrome
Etiology:
- megadose pyridoxine, 2-6 grams/day (RDA is 2 mg/day)
Pathology:
1) large fiber sensory loss
2) intoxication & death of dorsal root ganglion cells
3) peripheral & central axonal degeneration
3) axonal regeneration can occur is excessive pyridoxine is eliminated
Clinical manifestations:
1) ataxic peripheral neuropathy
2) proximal & distal parts of body affected simultaneously
3) sensory neuroapathy
a) Lhermitte's sign
b) paresthesia
c) dysethesia
4) sensory ataxia
5) pseudoathetosis without severe weakness
