Indications: - exertional angina pectoris Adverse effects: - neuropathy, hepatitis Laboratory: - perhexiline in serum/plasma Mechanism of action: - inhibits carnitine palmitoyltransferase (CPT-1 & CPT-2 - shifts myocardial substrate utilisation from long chain fatty acids to carbohydrates through inhibition of CPT-1 &, to a lesser extent, CPT-2 - results in increased glucose & lactate utilization & in increased ATP production for the same O2 consumption
