parathyroid adenoma
Epidemiology:
- ~75% women, usually 30's
Pathology:
- usually single, well circumscribed
- may deform esophagus or trachea
Microscopic pathology:
- cellular, chief cells & some oxyphil cells (see parathyroid gland)
- delicate capillary network
- microfollicles common
- rarely papillary
- minimal mitotic activity
- may see bizarre nuclei
- encapsulated, no capsular invasion
- no vascular invasion
- may have hemorrhage, cholesterol clefts, fibrosis
Genetics:
- t(11;11)(q13;p15) involving CCND1 with the parathyroid hormone (PTH) enhancer
Clinical manifestations:
- hypercalcemia
- hyperparathyroidism
