papain/urea (Panifil-White, Rystan, Kovea)
Tradenames: Panafil-White, Kovea, Rystan
Topical agent containing papain, urea with or without chlorophyll.
Topical papain withdrawn from US market in 2008 due to safety concerns & unapproved status. [4]
Panafil-White: urea 10%, papain 10% (30 g)
Indications:
1) acute & chronic skin lesions
2) varicose, diabetic & decubitus ulcers
3) postoperative wounds
4) pilonasal cyst wounds
5) carbuncles
6) traumatic wounds
Contraindications:
- infected wounds [3]
Dosage:
1) apply directly to lesion & cover with appropriate dressing
2) daily or twice daily changes of dressing are preferred
3) with each redressing, irrigate with normal saline
4) remove necrotic material
5) avoid hydrogen peroxide which may inactivate papain
Adverse effects:
1) not common (1-10%)
- transient stinging, burning sensation
2) uncommon (< 1%)
- irritation (from enzymatic exudate)
