oxtriphylline [choline theophyllinate] (Choledyl)

Tradename: Choledyl. (61% anhydrous theophylline). Indications: - reversible bronchospasm - symptomatic treatment of asthma - chronic bronchitis Contraindications: - uncontrolled arrhythmias Caution: 1) peptic ulcer disease 2) hyperthyroidism 3) congestive heart failure 4) COPD 5) liver disease pregnancy-category C safety in lactation ? Dosage: 1) adults: 4.7 mg/kg PO TID; administer sustained release every 12 hours 2) children: a) 1-9 years: 6.2 mg/kg every 6 hours b) 9-16 years: 4.7 mg/kg every 6 hours Tabs: 100 & 200 mg. Sustained release: 400 mg, 600 mg. Elixir: 50 & 100 mg/5 mL. Sustained action: Choledyl SA: - 400-600 mg PO BID Tabs: 400 & 600 mg. Pharmacokinetics: elimination: liver Adverse effects: 1) not common (1-10%) - tachycardia, nervousness, restlessness, nausea/vomiting 2) uncommon (< 1%) - insomnia, irritablity, seizures, rash, gastric irritation, tremor, allergic reaction Drug interactions: 1) smoking alters metabolism 2) cimetidine & possibly ranitidine 3) rifampin 4) phenytoin 5) barbiturates 6) ciprofloxacin 7) erythromycin Mechanism of action: - bronchodilator
