non-Q-wave myocardial infarction

Pathology: - patients with non-Q-wave MI tend to develop platelet-rich (white) subocclusive thrombi in contrast to larger, occlusive erythrocyte-rich thrombi of Q-wave MI Laboratory: - markers of myocardial infarction - diagnosis is commonly made only retrospectively on the basis of elevated cardiac enzyme levels after the event Special laboratory: - electrocardiogram - not associated with evolution of new Q waves Management: - non-Q-wave MI tends to respond more favorably to thrombolytic therapy - also see myocardial infarction Notes: - the diagnosis of non-Q-ave MI is often difficult to make soon after the event & is commonly made only retrospectively on the basis of elevated cardiac enzyme levels
