minimum data set (MDS) for nursing home residents

A 4 page form consisting of 16 sections required by the Omnibus Budget Reconciliation Act (OBRA) for all nursing home residents within 14 days of admission, with selected sections to be updated quarterly. The 16 sections include: 1) demographic & background information - includes advance directives 2) cognitive patterns a) delirium b) dementia 3) communication/hearing patterns 4) vision patterns 5) physical functioning a) activities of daily living (ADL) b) functional rehabilitation potential c) falls 6) continence a) urinary incontinence b) indwelling catheter 7) psychosocial well-being - feelings about self & social relationships 8) mood & behavior a) depression b) anxiety c) behavioral problems 1] wandering 2] verbal abuse 3] physical aggression 9) activity pursuit patterns a) activity/inactivity b) lack of participation 10) disease diagnoses 11) health conditions 12) oral/nutritional status a) nutritional status b) feeding tubes c) dehydration/fluid maintenance 13) oral/dental status -> dental care 14) skin condition - pressure ulcers 15) medication use -> psychotropic drug use 16) special treatments & procedures a) dialysis b) intravenous medications c) physical restraints Also see quality indicators in nursing homes
