Lugol's solution (iodine/iodide)
5% iodine/ 10% iodide.
Contraindications:
Pregnancy category: D
Adverse effects:
- not common (> 10%)
- angioedema, fever, headache, urticaria, acne, goiter with hypothyroidism, metallic taste, GI upset, cutaneous & mucosal hemorrhage, eosinophilia, arthralgia, lymph node enlargement, soreness of teeth & gums, rhinitis
Mechanism of action: Iodide limits its own transport & inhibits synthesis of iodotyrosine & iodothyronine. Intracellular iodide concentration appears to be the major determinant. This phenomenon is known as the Wolf-Chaikoff effect (1948). The iodine is reduced to iodide in the intestine before absorbtion.
