irradiation of cellular blood products
Indications:
Prevention of graft versus host disease (GVHD)
1) well established risk
- recipeints of allogeneic or autologous bone marrow transplant or stem cell transplants
- patients receiving aggressive chemotherapy &/or radiation therapy
- transfusions from blood relatives
- HLA-matched platelet transfusions
- congenital immunodeficiency syndromes
- premature infants
- intrauterine transfusions
- neonatal exchange transfusions
2) risk less well established
- recipients of solid organ transplants
- nearly all cases of GVHD are caused by engraftment of lymphocytes from the donor organ rather than from blood products
- HIV infection
- transfusion associated GVHD has not been reported in patients with AIDS
