dipivefrin ophthalmic (Propine)

Tradename: Propine. Indications: 1) open-angle glaucoma 2) secondary glaucoma 3) ocular hypertension 4) low-tension glaucoma 5) pseudoexfoliative glaucoma Contraindications: angle-closure glaucoma Dosage: 1 drop OU BID. 0.1% Adverse effects: 1) not common (1-10%) - headache, burning, stinging, ocular congestion, photophobia, mydriasis, blurred vision, ocular pain, conjunctival follicles, blepharoconjunctivitis, macular edema in up to 30% of patients with aphakia 2) uncommon (< 1%) - arrhythmias, hypertension Mechanism of action: 1) prodrug of epinephrine 2) stimulates alpha &/or beta adrenergic receptors 3) increases flow of aqueous humor 4) 4-11 times as potent as epinephrine in decreasing intraocular pressure 5) 5-12 times as potent as epinephrine in producing mydriasis
