diabetics who become pregnant

Special laboratory: - funduscopic examination by ophthalmologist - during preconception planning [1] - each trimester; continue for 1 year postpartum* [1] * postpartum frequency not specified [1] Complications: 1) maternal risks - pre-eclampsia - polyhydramnios - preterm labor - progression of existing diabetic retinopathy > diabetic nephropathy [9] - tight glycemic control may increase risk for diabetic retinopathy [1,9] 2) fetal risks - increased risk of major fetal malformations in the 1st 12 weeks of gestation - intrauterine death - acute respiratory distress syndrome - hypoglycemia - hypocalcemia - polycythemia - hyperviscosity - hyperbilirubinemia - cardiomyopathy - macrosomia - increased risk of early-onset cardiovascular disease in offspring (RR=1.3) [6] Management: 1) aggressive control of blood glucose - HgbA1c < 6.0% is optimal - continuous glucose monitoring in pregnant women with type 1 diabetes is associated with better neonatal outcomes [5] 2) type 2 diabetes a) switch from oral agents to insulin b) metformin & glyburide OK (pregnancy catergory B)* c) metformin appears to be a reasonably safe alternative to insulin for patients who refuse insulin [7] d) sulfonylureas, GLP-1 receptor agonists, dipeptidyl peptidase-4 inhibitors, or flozins not associated with increased risk of birth defects [8] 3) use bile acid sequestrant (colesevelam) for treatment of hypercholesterolemia [1] - statin contraindicated * oral hypoglycemic agents formerly contraindicated during pregnancy [1,2,3]
