clot lysis time
Normal range:
1) whole blood: lysis in 48-72 hours [2]
2) Thompson: lysis in 2-7 hours [2]
3) euglobulin: lysis > 60 minutes [1]
Clinical significance:
Whole blood:
1) lysis in < 24 hours indicates significant fibrinolysis
2) lysis in < 2 hours indicates severe fibrinolysis
Increases:
1) aminocaproic acid
Decreases:
1) fibrinolysis*
2) fibrinolytic therapy
3) chronic liver disease (50% of patients)
4) alpha-2 plasmin* May be normal in DIC if plasminogen has been depleted.. inhibitor [3]
* May be normal in DIC if plasminogen has been depleted
Interferences:
1) affected by plasma levels of:
a) plasminogen activator
b) plasminogen
c) fibrinolytic inhibitor
2) if fibrinogen is < 50 mg/dL, clot may be difficult to see
Specimen:
1) whole clotted blood (37 degrees C)
2) diluted whole blood (Thompson)
