cholestatic hepatitis

Etiology: - viral hepatitis - hepatitis A - hepatitis B - hepatitis C - pharmaceutical agents - alcoholic hepatitis - congestive heart failure - chronic pancreatitis - Hodgkin's disease - fatty liver Clinical manifestations: - jaundice - pruritus - hepatomegaly - fatigue - indigestion Laboratory: - serum direct bilirubin is elevated - serum bile acids elevated 10-20 fold - serum alkaline phosphatase is elevated - serum gamma-glutamyltransferase is elevated - serum 5'nucleotidase is elevated - serum ALT is moderately elevated - serum cholesterol is elevated Special laboratory: - abdominal ultrasound Management: - treat underlying etiology - nutrition a) low fat, low cholesterol diet b) vitamin supplementation (especially fat-souble vitamins) - cholestyramine may help reduce itch (see cholestatis)
