Tradename: Zostrix, Qutenza.
Epidemiology:
- capsaicin is the spice that make hot peppers 'hot'
- the Carolina Reaper is one of the hottest peppers in the world [9]
Indications:
- temporary relief of pain due to:
a) rheumatoid arthritis
b) osteoarthritis
c) neuralgia (diabetic, shingles, post Herpetic neuralgia)
d) musculoskeletal pain [4]
- minor sprains [6]
* on list of medications to avoid; limited effectivness [8]
Dosage:
- apply up to QID
- Qutenza must be applied to the skin by a healthcare professional
- it can be used for 60 minutes, once every 3 months [5]
Cream: 0.025%, 0.075%. 45 g
Transdermal patch Qutenza: 8%
Adverse effects:
1) burning of skin & mucous membranes
2) stinging
3) erythema
Mechanism of action:
- capsaicin activates TRPV1 in your mouth & through the GI tract signaling a burning sensation [9]
- depletion of substance P
Notes:
- Qutenza cost: $700/treatment (2010)
