aurothioglucose [gold] (Solganal)
Tradename: Solganal. [gold]. Rheumatoid arthritis: starting dose: 10 mg IM weekly. average dose: 50 mg IM monthly If no improvement after 1 g, reassess use of gold.
Pharmacokinetics:
1) gold content is 50%
2) elimination 1/2 life varies with dose
3) 70% excreted in urine, 30% in feces
Adverse effects:
- rash, may be localized
Toxicity: may be manifested by eosinophilia.
Drug interactions:
- may increase phenytoin levels
Mechanism of action:
1) supresses of prevents arthritis & synovitis
2) taken up by macrophages resulting in inhibition of phagocytosis & perhaps lysosomal activity
3) decreases serum levels of rheumatoid factor & immunoglobulins
4) may decrease synovial inflammation & retard cartilage & bone destruction
