aortic subvalvular stenosis
Classification:
- two major types:
1) discrete or membranous
2) fibromuscular (fixed or dynamic) (hypertrophic cardiomyopathy)
Etiology: congenital malformation
Pathology:
1) commonly associated with aortic regurgitation
2) high pressure gradient from left ventricle to aorta
Clinical manifestations:
1) prominent left ventricle
2) reduced pulse pressure
3) no ejection click
4) arterial thrill when stenosis is severe
5) hypertrophic obstructive cardiomyopathy
a) ejection murmur at left sternal border
b) prominent pulses with a bifid quality
Radiology:
1) 2-D & doppler echocardiography
2) left heart cardiac catheterization
