advanced sleep-phase syndrome; familial advanced sleep-phase syndrome (FASPS)
Epidemiology:
- common in the elderly
Pathology:
- circadian rhythm sleep-wake disorder
Genetics:
- associated with defects in CSNK1D
- associated with defecs in PER2
Clinical manifestations:
1) persistent early onset of sleep (8-9 PM)
2) early morning awakening (3-5 AM)
3) individuals are 'morning larks' with a 4 hours advance of the sleep, temperature & melatonin rhythms
4) early sleep patterns are not associated with shortened sleep duration
- sleep duration > 7 hours
Differential diagnosis:
- depression
Management:
- reassurance & patient education
- improved sleep hygiene may be beneficial
a) systematic gradual advancement of bedtime
b) bright light therapy early in the evening
- typical indoor lighting is inadequate
- sunlight or commercially available light box is needed
