Indications:
- used in conjunction with metyrapone stimulation test
Reference interval:
1) adult
a) male: 3.0-10.0 mg/day (8.3-27.6 umol/day)
b) female: 2.0-8.0 mg/day (5.5-22.1 umol/day)
c) 2.0-6.5 mg/g of creatinine (0.62-2.03 mmol/mol creatinine)
2) child: 1.0-5.6 mg/day (2.8-15.5 mg/day)
3) infant: (0-1 year): 0.5-1.0 mg/day (1.4-2.8 umol/day)
Increases:
1) pharmaceutical agents
a) in-vivo effects
-> ACTH, cortisone acetate, gonadotropins, hydrocortisone, trilostane
b) chemical interferences
-> acetazolamide, cephalothin, cefoxitin, chloral hydrate, chlordiazepoxide, chlorpromazine, colchicine, digitalis, erythromycin, fructose, glutethimide, iodides, ketoprofen, meprobamate, methenamine, methicillin, methyprylon, oleandomycin, paraldehyde, phenothiazines, spironolactone, troleandomycin
2) clinical disorders
a) Cushing's syndrome
b) stress or any acute illness
c) obesity
d) pregnancy
e) severe hypertension
f) acromegaly
g) thyrotoxicosis
Decreases:
1) pharmaceutical agents
a) in-vivo effects
-> corticosteroids, dextropropoxyphene, estrogens, medroxyprogesterone, meperidine, morphine, oral contraceptives, pentazocine, phenytoin
b) chemical interferences
-> carbamazepine, reserpine (high doses), thiazides
2) clinical disorders
a) Addison's disease
b) congenital adrenal hyperplasia (adrenogenital syndromes)
c) hypopituitarism
d) hypothyroidism
Method:
- colorimetric (Porter-Silber)
Specimen:
1) urine, 24 hours
2) preserve with boric acid & refrigerate
3) stable for at least 2 weeks & for longer periods if frozen at -20 degrees C
Notes:
- test has largely been replaced by serum cortisol & urine cortisol levels
