Indications: - syphilis Clinical significance: - sensitivity: - primary: 77-87% - secondary: 100% - tertiary: 37-94% - false positives: (titers rarely exceed 1:8) a) connective tissue diseases 1] lupus erythematosus 2] rheumatoid arthritis b) infections 1] infectious mononucleosis 2] leprosy 3] malaria 4] Mycoplasma pneumonia c) pregnancy d) addiction e) some immunizations f) advanced age (10% of patients > 70 years of age) - other antibodies found in patients with false + VDRL: a) antinuclear antibodies b) antithyroid antibodies 1] anti-thyroid peroxidase 2] anti-thyroglobulin c) antimitochondrial anatibodies d) rheumatoid factor e) Donath-Landsteiner antibody (paroxysmal cold hemoglobinuria) f) cryoglobulins Management: - a positive VDRL warrants - treponemal serologic test for syphilis
