nonallergic vasomotor rhinitis

Etiology: 1) smoke 2) air pollution 3) perfumes 4) detergents & soaps 5) solvents or fumes 6) changes in temperature, light, or atmospheric pressure - cold air 7) excercise 8) emotion Clinical manifestations: 1) nasal congestion 2) clear rhinorrhea 3) postnasal discharge 4) sensitivity to irritants & temperature changes 5) perennial symptoms Laboratory: 1) normal serum IgE 2) absence of nasal eosinophils Management: 1) nasal ipratropium - effective for rhinorrhea, but not for nasal congestion 2) nasal antihistamines &/or nasal glucocorticoids for nasal congestion in addition to rhinorrhea 3) oral decongestants (pseudoephedrine) older recommendation 4) nasal saline irrigation is an adjuvant therapy
