trifluridine (Viroptic)
Tradename: Viroptic.
Indications:
1) treatment of primary keratoconjunctivitis & recurrent epithelial keratitis caused by Herpes simplex virus types 1 & 2
2) effective in treatment of epithelial keratitis resistant to idoxuridine
Dosage:
1) 1 drop every 2 hours while awake, up to 9 drops/day
2) continue with 1 drop every 4 hours for an additional 7 days after re-epithelialization of corneal ulcer
3) do NOT exceed 21 days of therapy
Solution: 1%. (7.5 mL).
Adverse effects:
1) not common (1-10%)
- burning, stinging
2) uncommon (< 1%)
- palpebral edema, epithelial keratopathy, keratitis, stromal edema, increased intraocular pressure, hyperemia, hypersensitivity
Mechanism of action:
1) replaces thymidine in viral DNA synthesis
2) causes production of defective viral proteins
3) reversibly inhibits thymidylate synthetase
