transmyocardial laser revascularization
Criteria:
1) LV ejection fraction > 30%
2) angina refractory to optimal medical therapy
3) reversible ischemia of the left ventricular free wall & coronary artery disease corresponding to region of myocardial ischemia
4) not be amenable to coronary artery bypass graft (CABG) or percutaneous transluminal coronary angioplasty (PTCA)
5) angina either due to
a) severe diffuse disease
b) lack of suitable targets for complete revascularization
c) lack of suitable conduits for complete revascularization
