telmisartan (Micardis)

Tradename: Micardis. Indications: - treatment of hypertension* - chronic heart failure - left ventricular systolic dysfunction - diabetic nephropathy - nondiabetic proteinuric nephropathy - myocardial infarction [3] - ischemic stroke [3] ; * Telmisartan superior in controlling 24-hour blood pressure compared with other ARBs [4] * Telmisartan with similar cardiovascular outcomes to those of other ARBs [4] Dosage: 20-80 mg PO QD Tabs: 20, 40, 80 mg Pharmacokinetics: 1) bioavailablity 42-58%, orally active (not prodrug) 2) peak effect 0.5-1 hour 3) metabolized via conjugation to inactive metabolites 4) terminal 1/2life of 24 hours 5) 97% excreted in feces 6) clearance: 800 mL/min Adverse effects: - see ARB Drug interactions: 1) weak inhibitor of cyt P450 2C19 2) telmisartain may increase levels of: - digoxin, Li+, 3) K+ & K+-sparing diuretics may increase risk of hyperkalemia 4) warfarin ? Mechanism of action: - angiotensin II receptor antagonist - angiotensin II receptor type 2 & 4-stimulating antihypertensive see ACE inhibitors vs angiotensin receptor blockers (ARB)
