sodium thiosulfate (Sodothiol)
Indication:
1) cyanide poisoning
- used in combination with sodium nitrite in the cyanide antidote pack
2) arsenic poisoning
Dosage:
1) cyanide & nitroprusside (poisoning)
a) 2.5-5 mL/min IV to a maximum of 50 mL
b) see cyanide antidote pack
2) arsenic poisoning
a) 1 mL on 1st day
b) 2 mL on 2nd day
c) 3 mL on 3rd day
d) 4 mL on 4th day
e) 5 mL on alternate days, thereafter
Injection: 100 mg/mL (10 mL)
Pharmacokinetics:
1) distributes to extracellular fluid
2) 1/2life 30-40 minutes
3) excreted unchanged in the urine
Adverse effects:
- not common (1-10%)
- hypotension, coma, CNS depression (secondary to thiocyanate toxicity), psychosis, tinnitus, confusion, weakness, contact dermatitis, local irritation
Poisoning: Contact Regional Poison Control Center as needed for consultation on use as an antidote
