Indications: - evaluation of acute pancreatitis Reference values: - Male & Female: Serum: 30- 110 U/L Increases: 1) acute pancreatitis a) serum levels rise within 6-48 hours after onset of acute pancreatitis in about 80% of patients b) values in excess of 4 times the upper limit of normal are highly suggestive of pancreatitis c) activity of amylase in serum generally returns to normal within 3-5 days in patients with milder forms of acute pancreatitis d) elevated serum amylase persisting more than 3-5 days suggests pancreatic pseudocyst or pancreatic necrosis [4] e) use in conjunction with serum lipase improves specificity 2) mumps, parotitis 3) intestinal obstruction 4) intestinal ischemia, intestinal infarction, appendicitis 5) renal failure 6) also see hyperamylasemia 7) pharmaceutical agents: - in vivo effects - cholinergic agents, ethanol, narcotics Decreases: - pharmaceutical agents: - chemical interferences - citrate, oxalate, fluorides Principle: - see amylase in serum & urine Specimen: No special patient preparation is necessary. Sample preparation: Collect the specimen by standard venipuncture technique. Use Lithium or sodium heparin as an anticoagulant for plasma specimens. Plasma values are approximately 20 uL higher than serum values. Urine: Collect timed or random specimens by standard laboratory procedures. Timed specimens are preferred: DO NOT USE URINE CONTAINING CHELATING AGENTS. (e.g. citrates, oxalates, or EDTA) Special Precautions: Serum or Plasma: See Sample Dilution for grossly lipemic specimens. Refrigerate or freeze samples that cannot be analyzed immediately. Do not use calcium-chelating anticoagulants such as oxalates, citrated, & EDTA. Minimum sample size 0.5 milliliters with an optimum size of 1.0 milliliter or larger.
