3) serum urea nitrogen (BUN) > 28 mg/dL (10.0 mmol/L)
4) serum glucose > 252 mg/dL (14.0 mmol/L)
5) pulse > 120/min
6) serum bicarbonate < 20 mmol/L
7) involvement of > 10% of body area
