reactive arthritis
Joint inflammation alone without other features of Reiter's syndrome.
Etiology:
1) induced by a host response to an infectious agent
a) upper respiratory tract infection (coxitis)
b) gastrointestinal infection (infectious diarrhea)
c) urogenital infection
2) etiologic infectious agents:
a) Salmonella enteritidis
b) Shigella flexneri
c) Yersinia enterocolitica
d) Campylobacter jejuni
e) Chlamydia trachomatis
f) Ureaplasma urealyticum
g) Mycobacterium tuberculosis (Poncet's disease)
3) associated disorders
a) inflammatory bowel disease
b) HIV1 associated with increased severity of reactive arthritis [2]
Epidemiology:
- equal distribution between sexes
Pathology:
1) joint is aseptic
2) disease is generally self-limited
3) joint destruction is uncommon
4) bacterial antigens are found in affected joints
Genetics:
- HLA-B27 positive (80%)
Clinical manifestations:
1) may manifest 2-3 or 6 weeks after a GI infection or GU infection
- it is common for precipitating infection to have resolved prior to onset of reactive arthritis [2]
- sterile urethritis, prostatitis, cervicitis, salpingitis, balanitis
2) classic triad of arthritis, conjunctivitis & urethritis/cervicitis in 1/3 [2]
3) monoarthritis or oligoarthritis (maybe not polyarthritis) [2]
a) generally large joints, weight-bearing joints, especially knee, ankle
b) distribution is asymmetric
c) joint effusions may be massive
d) usually non-erosive [2]
4) popliteal (Baker's) cysts are common
5) tenosynovitis
6) dactylitis
7) enthesopathy (enthesitis)
a) Achilles' tendonitis
b) plantar fasciitis
8) arthritis generally self-limited & non-erosive [2]
a) some patients have a relapsing, remitting course
b) attacks are generally months in duration
c) may be chronic
9) symptoms of Reiter's syndrome, including sacroiliitis & spondylitis may develop [2]
10) mucocutaneous manifestations
a) psoriasis-like skin lesions
b) mouth ulcers, red eyes
c) keratoderma blennorrhagicum (psoriaform lesions on palms, soles & toes) [2]
d) circinate balanitis (shallow, moist, serpinginous ulcers with raised borders)
e) urethritis, cervicitis
11) ocular manifestations:
- conjunctivitis more common than uveitis [2]
- photophobia
12) may be more severe in patients with HIV1 infection, especially when HLA-B27 positive [2]
Laboratory:
1) GenProbe
a) Neisseria gonorrhoeae DNA
b) Chlamydia trachomatis DNA
2) serology
a) HIV serology
b) Salmonella serology
c) Yersinia serology
d) Campylobacter serology
e) Neisseria serology
3) urethral cultures for Chlamydia
4) throat cultures for Streptococcus
5) urinalysis: urine protein, WBC, RBC
6) synovial fluid analysis:
- rule out septic arthritis
- WBC: 15,000/uL with 70% monocytes (MKSAP17 case)
- gram stain negative, no crystals [2]
7) HLA-B27 positive (80%)
Radiology:
- radiographs may show localized osteopenia [2]
- radiograph of sacroiliac joint may show sacroiliitis [2]
Differential diagnosis:
- coxitis (postinfectious arthritis)
- Chikungunya
- Dengue
- parvovirus B19
- Lyme disease
- Behcet syndrome: genital ulcers, not urethritis, not infectious
Management:
1) specific antibiotic if etiologic agent can be identified
a) reactive arthritis due to Chlamydia
1] doxycycline first line, azithromycin for pregnant women
- macrolides, fluoroquinolones, tetracyclines (doxycycline) active against Chlamydia trachomatis
2] treatment of patient & sexual partner indicated
3] long-term combination antibiotic treatment may be of benefit [2]
b) reactive arthritis due to enteric infection
1] may be self-limited
2] infection often resolved before patient diagnosed with reactive arthrtitis
3] antibiotics not useful unless active infection is present [2]
c) antibiotics usually not helpful if etiologic agent not identified [2]
2) NSAIDs (indomethacin first line)
- with caution in patients with inflammatory bowel disease
3) intraarticular glucocorticoids may be effective after septic arthritis is ruled out
4) disease-modifying anti-rheumatic drug (DMARD) if arthritis persists for 3-5 month
a) sulfasalazine benefits some patients unresponsive to NSAIDs alone
b) methotrexate, start with 7.5 mg PO QD
c) TNF-alpha inhibitor
5) tetracycline (doxycycline) for 3 months may diminish the duration & severity if secondary to Chlamydia trachomatis
6) prognosis: resolution of symptoms generally within 6 months [2]
