propantheline (Pro-Banthine)
Tradename: Pro-Banthine.
Indications:
- adjunctive therapy for peptic ulcer disease
- irritable bowel disease
- used in conjunction with H2 receptor antagonists in the treatment of Zollinger-Ellison syndrome
- urinary incontinence [4]
Dosage: 15 mg PO 30 min QAC & QHS.
Tabs: 7.5 & 15 mg.
Pharmacokinetics:
1) quaternary ammonium drug with variable GI absorption, depending upon presence of food
2) distribution is unknown, but does NOT readily penetrate CNS
3) elimination 1/2life is 9 hours
Adverse effects:
1) common (> 10%)
- constipation
- decreased sweating,
- dry mouth, skin, nose or throat
2) less common (1-10%)
- difficulty swallowing
3) uncommon (< 1%)
- rash, confusion, increased intraocular pressure, bloating, blurred vision, headache, loss of memory, nausea/vomiting, weakness, tiredness, urinary retention, tachycardia, drowsiness, nervousness, insomnia
4) cholinergic effects:
- dry mouth
- constipation
- urinary retention
Mechanism of action:
1) not fully understood
2) may act as nonselective smooth muscle relaxant
3) little or no muscarinic activity, except at very high doses
