post-cholecystectomy syndrome

Etiology: - postoperative complication of cholecystectomy Pathology: - continuously increased bile flow into the upper GI tract - may contribute to gastritis, GERD & esophagitis - may cause diarrhea & colicky lower abdominal pain - sphincter of Oddi dysfunction - post-surgical adhesions. - biliary microlithiasis Clinical manifestations: - dyspepsia - nausea, vomiting - flatulence - bloating - diarrhea - right upper quadrant pain Special laboratory: - abdominal ultrasound - ERCP may be diagnostic & therapeutic Management: - treatment of diarrhea - fiber - cholestyramine - antispasmodics for colic - proton pump inhibitor for gastritis, GERD & esophagitis
