Indications:
- recovery from PCP infection
- HIV infection
- HIV1 infection with CD4 counts < 200/mm3
- HIV1 infection with < 14% CD4/total lymphocytes
- *HIV1 infection with unexplained fevers lasting < 2 weeks
- *HIV1 infection with oral candidiasis
- any AIDS-defining condition or Kaposi's sarcoma
