Indications:
- evaluation of hyperaldosteronism
Reference interval:
1) supine: 2-5 ng/dL (60-140 pmol/L)
2) standing: 7-20 ng/dL (194-555 pmol/L) [1]
* assumes normal sodium intake
Increases:
1) primary hyperaldosteronism
a) bilateral adrenal hyperplasia
b) aldosterone-secreting adrenal adenoma
2) secondary hyperaldosteronism
a) laxative abuse
b) diuretic abuse
c) heart failure
d) cirrhosis of the liver with ascites
e) nephrotic syndrome
f) idiopathic cyclic edema
g) Bartter' syndrome
h) hypervolemia due to hemorrhage
i) renal juxtaglomerular hyperplasia with K+ wastage
j) renal hemangiopericytoma producing renin
k) malignant hypertension of renal origin
l) thermal stress
m) pregnancy
n) starvation after 10 days
o) COPD
3) drugs
- angiotensin, estrogens, laxatives, loop diuretics, metoclopramide, oral contraceptives, spironolactone, thiazides, K+, serotonin reuptake inhibitors (SSRI),
Decreases:
- Liddle syndrome
- drugs
- aminoglutethimide, licorice, direct renin inhibitors NSAIDs
- alpha-adrenergic receptor agonists
- beta-adrenergic receptor antagonists [1]
- ACE inhibitors, dihydropyridine calcium channel blockers

Specimen:
- plasma (heparin or EDTA) or serum
a) store frozen
b) stable for 2 years at -20 degrees C in airtight container
