peritoneal carcinomatosis

Etiology: 1) ovarian cancer - women with peritoneal carcinomatosis of unknown primary have ovarian cancer until proven otherwise 2) salpingo-oophorectomy reduces risk of cancer in patients with BRCA1 mutations; however, peritoneal carcinoma histologically indistinguishable from ovarian cancer may develop Genetics: - associated with BRCA1 mutations Clinical manifestations: - weight loss - ascites [1] Laboratory: - in patients with ovarian cancer or unknown primary, serum CA125 can be useful in monitoring response to therapy Management: 1) if no primary neoplasm is identified, treat women as if they have stage III ovarian cancer - surgical debulking including salpingo-oophorectomy, hysterectomy, omentectomy, appendectomy, lymphadenectomy followed by chemotherapy [1] 2) prognosis: 5 year survival is 15-20% [1]
