paraneoplastic subacute sensory neuronopathy

Etiology: 1) mediator: unknown 2) associated neoplasms: lung, especially small cell Pathology: 1) inflammation & dorsal root degeneration in dorsal root ganglia 2) secondary axonal loss Clinical manifestations: 1) evolution over weeks to months 2) may precede other manifestations of malignancy 4) severe sensory loss with areflexia & ataxia 5) paresthesias, pain 6) ataxia 7) may be purely painful or purely ataxic Laboratory: - anti-Hu antibodies (50%) - nerve biopsy Special laboratory: - nerve conduction studies
