orthopedic surgery

Indications: - bone disease &/or joint disease not amenable to medical treatment * many common elective orthopedic surgical procedures lack strong evidence of effectiveness [2] - carpal tunnel release & total knee arthroplasty appear to be superior to nonsurgical management alone - total hip arthroplasty & arthroscopic meniscal repair have not been compared with nonsurgical management in randomized trials - arthroscopic partial meniscectomy, rotator cuff repair, subacromial decompression, reconstruction of the anterior cruciate ligament, lumbar spine decompression, & lumbar spine fusion appear to be no better than nonsurgical treatment [3] Complications: - older age, diabetes mellitus, baseline renal insufficiency, & other comorbidities are associated with increased risk for acute renal failure after orthopedic surgery - acute renal failure, in turn, is associated with increased mortality [1] - hyponatremia, SIADH in elderly (30%) Notes: - see surgery
