Deficiency of amniotic fluid Etiology: - premature rupture of placental membranes - fetal oligouria Epidemiology: - 4% of pregnancies Pathology: - compression of the fetal abdomen limiting movement of the diaphragm - pulmonary hypoplasia - fetal heart conduction abnormalities - umbilical cord compression - poor tolerance of labor - fetal acidosis Clinical manifestations: - meconium staining of the amniotic fluid - lower Apgar scores Physical examination: - check blood pressure Laboratory: - antinuclear antibody screen for SLE - liver function tests - complete blood count - low platelet count (HELLP syndrome) - urine protein for proteinuria - serum uric acid for elevation Special laboratory: - fetal ultrasound Complications: - high mortality Management: - bedrest - oral hydration - OB-Gyn consult
