Fisher syndrome (Miller-Fisher syndrome, anti GQ1b antibody syndrome)

The most common variant of Guillain-Barre syndrome, accounting for 5% of all cases. (also see NINDS Miller Fisher Syndrome Information Page) Etiology: - may be associated with infections, autoimmune & neoplastic disorders Pathology: - neuromuscular transmission defects in association with anti GQ1b IgG antibody Clinical manifestations: 1) ophthalmoplegia 2) ataxia 3) areflexia 4) little weakness Laboratory: - anti GQ1b IgG antibody titer (generally elevated) Management: - mostly, acute, self-limiting syndrome - anecdotal evidence of benefit with immunotherapy
