mental status examination (MSE)

A descriptive inventory of behavior & a method by which to systematically organize & record observations which describe a patient's behavior. Indications: - standardized cognitive testing for elderly who report short-term memory loss & deteriorating independence - allows for screening in multiple domains, & for deficits that may assist with localization & diagnosis [2] Function: 1) aid in understanting a patient's psychological state 2) help in diagnosis 3) baseline for comparison of future assessments 4) written, descriptive record of the patient Components: 1) appearance -> an individualized & vivid portrait of the patient 2) behavior a) posture b) gait c) tremors 3) speech a) rate b) volume 4) attitude a) indicates how patients may get along with others b) (e.g. suspicious, domineering, submissive, provocative uncooperative) 5) feelings a) affect b) mood 6) perception a) illusions b) hallucinations 7) orientation -> orientation to time, place, person 8) thinking a) level of consciousness b) continuity of thinking c) obsessions, compulsions d) phobias e) depersonalization & derealization f) suicidal or homicidal thinking g) delusions h) ideas of reference or influence 9) higher intellectual function a) memory b) attention & concentration c) general information, intelligence d) calculation e) abstraction f) language g) visual/spacial skills h) judgement, insight
