Notes:
- Medicaid beneficiary (rather than the agency) hires, trains, supervises, & manages workers who provide personal care & related services for the beneficiary
- in most cases a family member or friend is paid to provide direct personal care to the beneficiary
- states are required to arrange for a system of support to help the Medicaid beneficiary
