lung cancer

Primary lung cancers are predominantly lung carcinomas. Etiology: - risk factors (see lung carcinoma) Epidemiology: - leading cause of cancer-related death world-wide [5] - incidence declining in men & young adults in association with declining rates of smoking 2005-2009 [7] Pathology: 1) lung small cell carcinoma (14%) a) occurs almost exclusively in smokers b) invariably metastatic on presentation 2) lung squamous cell carcinoma (20%) - tends to occur more centrally 3) lung adenocarcinoma (38%) a) most common type in non-smokers b) tends to occur more peripherally 4) lung bronchoalveolar carcinoma (lung adenocarcinoma, 3%) -> low-grade carcinoma frequently presenting as patchy infiltrate 5) lung large cell carcinoma (3%) 6) lung adenosquamous carcinoma (0.6-2.3%) 7) lung bronchial carcinoid (1-2%) 8) sarcomatoid carcinoma (0.3%) 9) mesothelioma 10) lung mucoepidermoid carcinoma (0.1%) 11) lung adenoid cystic carcinoma (< 0.1%) 12) metastases a) lymph node metastases (83% local, 29% distant) b) liver metastases (73%) c) ipsilateral or contralateral lung metastases(8%) d) bone metastases(56%) e) brain metastases (37%) f) adrenal metastases (35%) g) kidney metastases (16%) [5] Clinical manifestations: (see lung carcinoma) Laboratory: - tissue diagnosis (histopathology) needed for diagnosis [5] - lung biopsy - needle aspiration rather than biopsy of lymph node diagnostic [5] - see ARUP consult [5] Special laboratory: (see lung carcinoma) Radiology: (see lung carcinoma) Complications: (see lung carcinoma) Management: (see lung carcinoma)
