Indications: - heart failure - fluid overload, edema, peripheral edema, pulmonary edema - markedly decreased renal function. oliguria - hypertension - hypercalcemia [4] * do not change loop diuretic to thiazide diuretic for hyperkalemia in patients with systolic heart failure [5] Monitor: - serum electrolytes, serum Mg+, serum Ca+2 annually [3] Adverse effects: 1) hyperuricemia 2) hypocalcemia 3) hypokalemia 4) ototoxicity 5) rash 6) vasculitis 7) diuretic resistance Mechanism of action: - site of action: loop of Henle - not as effective as thiazide for lowering blood pressure in most patients [2]
