Protocol for administration of insulin to control hyperglycemia. Daily basal insulin should also be included in the protocol. [1] blood glucose regular insulin (mg/dL) (units) < 60 give 1 vial D50W, or give orange juice 60-150 0 (do nothing) 151-200 2 201-250 4 251-300 6 301-350 8 351-400 10 > 400 call MD Notes: - daily basal insulin should be adjusted to minimize amount of sliding scale administered in the absence of episodes of hypoglycemia
