inappropriate sinus tachycardia

Epidemiology: - women in 2nd-4th decades of life - more common in healthcare professionals Clinical manifestations: - sinus tachycardia at rest - exagerated increases in heart rate with light activity - decrease in heart rate during sleep - palpitations, lightheadedness, dyspnea, fatigue, syncope Laboratory: - TSH in serum/plasma - complete blood count (CBC) - 24 hour urine catecholamines Special laboratory: - echocardiogram: structurally normal heart - HOLTER Differential diagnosis: - hyperthyroidism - anemia - pheochromocytoma - structural heart disease Management: - beta-blocker or calcium channel blocker - ivabradine
