ibandronate; ibandronic acid (Boniva, Bondronat)

Tradename: Boniva (FDA approved 2003) Generic FDA-approved 3/2012 [6] Indications: -> osteoporosis - diminishes risk of vertebral fractures - does not diminish risk of hip fracture [7] Contraindications: 1) renal insufficiency a) creatinine clearance < 30 mL/min b) serum creatinine > 2.3 mg/dL 2) uncorrected hypocalcemia (IV) Pregnancy category: C Dosage: 1) 2.5 mg PO QD [3,4] 2) 150 mg PO monthly [3,4] 3) 3 mg IV every 3 months (infuse over 15-30 seconds) Try to take on same day each month. +/- 1 week OK if average interval is one month [4] Do NOT eat or lie down for 60 minutes after taking monthly Boniva. Prefilled syringe 3mg/3mL Monitor: serum creatinine prior each IV dose Adverse effects: 1) arthralgia (8-9%) 2) back pain (7%) 3) hypertension 5-7% 4) abdominal pain 5-6% 5) GI distress < 5% - dyspepsia, nausea, constipation, diarrhea, gastritis 6) transient hypocalcemia Drug interactions: none reported Mechanism of action: 1) bisphosphonate 2) reduces absolute vertebral fracture risk 5% [4]
